This form is used to report invention or software made at CU Boulder and UCCS. It consists of two sections: Inventors and Invention. Once you have filled out the form and submitted the data, you'll have a chance to review (or edit if necessary), and finish the process by clicking a "Finish" button. After that you'll receive an confirmation email with the submitted form in a pdf document.

Important note: Each inventor's home address is required for patent applications and for distribution of licensing revenue. Please also provide a personal email address; revenue may be collected after an inventor has left the University. Please ensure personal email and home address information is on hand before proceeding.

Caution: Do NOT close browser windows during the process or all work will be lost.
